  Username 

Login Instructions

Visitors should use the Site Name of Guest and the Username of Test2.

To download the Excel file hover over the link, right click and choose "File Save As"

You should login each time to the benchmarking forms using the following process. If your site name is not listed or you do not know your Username please contact Keith McClanahan at 928.213.9767 or email keithmcc@facilityissues.com.

  • Site Name... Select your site from the dropdown menu
  • Username... Assigned to your benchmarking team leader by email from Facility Issues.

Data Adjustment Instructions 

When you select a Site Name and enter your User name, information that has previously been entered on a form will automatically fill in. If you want to change a value in a previously submitted form...
  • Login using your Site Name and Username (must be same Username as used for prior submittals)
  • Go to the form that you want to change your data input
  • Change the value, print the adjusted form for your records,  and click the submit button.
