Login Instructions

You should login each time to the benchmarking forms using the following process:

• Site Name... Select your site from the dropdown menu. For new registrants, your Site Name will be added to the drop down list at the close of each business day on the day you register. If your site is not listed by the following business contact email:

• Username... Will be provided to you by email.
  Caution - When entering multiple sites, you must return to this Login Screen and select the appropriate Site Name and input unique Username to continue your data input.

Data Adjustment Instructions 

When you select a Site Name and enter your user name, information that has previously been entered on a form will automatically fill in. If you want to change a value in a previously submitted form...

• Login using your Site Name and Username (must be same Username as used for prior submittals)
• Go to the form that you want to change your data input
• Change the value, print the adjusted form for your records,  and click the submit button
